Five years ago there was essentially one conversation to have about weight-loss medication in India, and for most people the answer was that it was unaffordable. Today there are four viable routes, prices that span an order of magnitude, and a genuinely difficult decision to make.

This guide compares every weight-loss injection currently available in India — Mounjaro, Wegovy, Ozempic and generic semaglutide — on the things that actually decide the outcome: what they do, what they cost, who they suit, and what happens after you stop.

The 2026 Landscape

Two events reshaped this market. Mounjaro launched in India in March 2025 and by October that year had become the country's top-selling drug by value. Then, on 20 March 2026, the Indian patent on semaglutide expired, and dozens of generic manufacturers entered within weeks. Novo Nordisk responded by cutting branded prices twice.

The practical result: the cheapest legitimate route to GLP-1 treatment in India now costs a small fraction of what the most expensive one does. That is a real choice, and it deserves more thought than picking the cheapest line on a price list.

How They Differ Mechanically

Three of the four are the same molecule. The real mechanical divide is between semaglutide and tirzepatide.

Side Effects Across the Class

The side-effect profile is broadly similar across all four, because three share a molecule and the fourth works through an overlapping pathway. Most are gastrointestinal, most noticeable around dose increases, and most settle as your body adjusts.

Who should not take any of them

Insulin and sulfonylureas need dose review

What Happens When You Stop

This applies identically to all four options and it is the part price comparisons never mention.

Appetite returns when the medicine wears off, and weight commonly returns with it. This is physiology, not a failure of resolve. It is also entirely plannable — a tapered step-down with protein targets and resistance training established beforehand holds results far better than stopping abruptly.
